In case a particular handout didn’t make it home, the following is a list that you can print out to send in or keep for your records:

Student Medical Form
Every band member should have one of these forms filled out. It includes a section for medical conditions that the staff should be aware of (such as allergies). It also has a section to give permission for medical treatment should the need arise.
